Flexible working request forms and letters
This pack helps an employer manage and process flexible working requests in an easy and efficient manner, without it becoming a protracted exercise.
This pack includes five documents aimed at helping an employer handle flexible working requests, including asking an employee to provide sufficient information in order that a decision can be reached and complying with employer legal obligations.
All decision outcome letters clearly communicate employee rights and responsibilities, including the next course of action available and employee right of appeal.
Pack contains:
Employee form: flexible working request form (employee completes and returns form, recording: reasons for request; entitlement to request flexible working; previous requests; personal circumstances; and working hours, days, role, location being requested).
Employer letters to send an employee:
- hearing invitation letter (to discuss and explore request)
- decision - agreement letter (detailing new work and employment terms)
- decision - decline letter (no change to current hours, role, location etc)
- decision - counter-offer letter (part-agreement or proposed alternatives)
