Medical Report Request Letter and Form
Want to know about an employee's current state of health?
This letter and form will help you request a medical opinion and prognosis from a GP, specialist, company doctor or occupational health advisor, all within a reasonable time frame.
The form includes questions to ask a medical practitioner in relation to:
- Current state of health and medical prognosis of the condition.
- Likelihood of a return to work in a current or another role.
- Any reasonable adjustments to aid a return to work.
- Qualification under the Disability Discrimination Act 1995.
These documents also provide sufficient space for you to add any additional information or questions, and confirms when you expect the report to be sent to you (in accordance with the Access to medical Reports Act 1988) and details relating to the employee's current role, responsibilities and physical activities.
