Medical Report Consent Form

To obtain consent for a medical report on an employee, use this form (with covering letter) to explain the request to the employee, obtain contact details, explain the employee's rights and confirm the employee's consent.

• The letter clearly explains to an employee why you are seeking a medical opinion.
• The form asks the employee to confirm GP, doctor and medical practice contact details.
• The form also records the employee's consent for you to obtain medical information from their GP, doctor and/or medical practitioner.
  
• The letter also advises employee rights under the Access to Medical Reports Act 1988.

The same letter and form can also be used when approaching your own company doctor or an occupational health advisor.
