Medical Home Visit Letters
Use these two letters to manage periods of long-term sickness absence.
These letters are aimed at helping an employer formally request a meeting with an employee (at their home, at work or at another agreed location) to discuss an employee's continued absence from work, the company's concerns and agree a plan of action to help facilitate an early return to work.
Combined these two letters form part of long-term sickness absence management activities and help to record discussions held and actions agreed, should pro-longed absence result in ill-health or incapability dismissal.
Meeting Invitation Letter
Use this letter to invite an employee to attend a meeting, on-site or at home, to:
- discuss continued sick leave, absence from work and payments during absence
- explore the reasons for absence and continued sickness
- outline your concerns and explore appropriate assistance to facilitate a return to work
- agree a plan of action going forward and obtain a medical report (if appropriate)
- inform an employee of their rights at the meeting and during absence
- confirm what actions are required by both employer and employee
Meeting Outcome Letter
Use this letter to confirm the outcome of the above meeting, including:
- confirming periods of and reasons for long-time absence and sick leave
- confirm payment of salary, wage, SSP and company sick pay during absence
- outline company concerns, assistance explored and specify medical opinion sought
- confirm any help which will be provided and any agreed actions
- what activities will be undertaken by either party and the next course of action
- confirmation of what, if any, action will result should long-term sickness absence continue
